Whiplash After a Car Accident
Whiplash is a neck injury caused by the head being thrown rapidly backwards and forwards — most often in a rear-end collision, but also in sport or a fall. The forces involved strain the muscles, ligaments and joints of the neck, and symptoms frequently take a day or two to appear.
What it feels like
- Neck pain and stiffness, usually worse the morning after the collision
- Reduced ability to turn or tilt the head
- Headaches, typically starting at the base of the skull
- Pain across the shoulders and into the upper back
- Pins and needles or numbness in an arm
- Dizziness, poor concentration, disturbed sleep or unusual fatigue
- Jaw pain, or difficulty chewing
Symptoms appearing a day or two later is normal and does not mean the injury is worse. It does mean that a check-up after a collision is worthwhile even if you felt fine at the scene.
Why early treatment matters
The evidence on whiplash is consistent on one point: staying gently active and returning to normal movement early is associated with better outcomes than resting and waiting. Prolonged immobilisation and collars are no longer recommended for most people. Getting assessed early means you can start moving safely and with a clear plan.
How physiotherapy helps
- Early gentle range-of-motion work to get the neck moving again within safe limits
- Manual therapy to the neck and upper back as symptoms allow
- Progressive strengthening of the deep neck and shoulder blade muscles
- Headache treatment where the upper neck is involved
- Graded return to driving, work, lifting and sport
- Laser therapy, electrotherapy, dry needling or massage therapy alongside active treatment
- Reassurance and clear information, which the research shows genuinely affects recovery
If you were in a car accident in Ontario
Physiotherapy after a motor vehicle collision is covered through your own auto insurer’s statutory accident benefits, regardless of who was at fault. A few practical points:
- You do not need a referral from a doctor to start physiotherapy.
- Report the collision to your insurer and ask for the accident benefits application. There are deadlines, so do this promptly.
- We complete the required treatment plan forms and submit them to your insurer, and we bill the insurer directly — so in most cases you pay nothing up front.
- Rates for accident benefits treatment are set by regulation and are the same at every clinic.
- Keep a note of your symptoms and the days you missed work. It helps both your recovery and your claim.

How long does recovery take?
Many people improve substantially within six to twelve weeks. A proportion take longer, particularly where there were symptoms in the arm, high initial pain, or previous neck problems. Your physiotherapist will reassess as you go rather than commit you to a fixed number of visits at the outset.
When to seek medical attention first
Go to an emergency department after a collision if you have severe neck pain with midline tenderness, weakness or numbness in the arms or legs, problems with balance or walking, loss of consciousness, worsening headache, repeated vomiting, or confusion.
Booking an appointment
You do not need a doctor’s referral to see a physiotherapist in Ontario. Some extended health plans ask for one before they will reimburse you, so it is worth checking your policy.
