- Visit our healthcare provider (e.g., physiotherapist) who will help you fill out the necessary forms (such as the OCF-23 or OCF-18 in Ontario)
OCF forms are official documents used in Ontario for auto accident injury claims. For example, OCF-23 is used for Minor Injury Guidelines (MIG), and OCF-18 is used for serious or complex injuries. Our healthcare provider will complete these forms for you.
